The fact that we are a capacity building service means that we see client’s only as much as necessary and then discharge – there are no maintenance services here!

The opportunity to engage in multidisciplinary collaboration– collaboration is crucial to the success of your intervention in disability – meet regularly with behaviour support therapists, speech therapists, OTs, support workers, support coordinators, GPs, exercise physiologists and physiotherapists.

Work from home, manage your own diary (attend personal appointments if you need), work independently but seek support and assistance from management when you need it.

 

Your responsibilities will include:

  • The successful applicant will provide mobile dietetic services to Active Ability’s clients.
  • Assess and provide support to clients in their own home, to carry out interventions that help them to achieve their goals, improve functional independence and quality of life
  • Provide face to face and occasional tele-support to clients in community settings of their choice, in their own home, group homes, day programs.
  • Providing dietetic and behaviour change advice to clients living with disability or chronic/complex conditions
  • Provide appropriately targeted education for clients, carers and staff as required
  • Ensure appropriate written and verbal communication with key stakeholders as required including timely reports and meal plans
  • Communicate and work efficiently and collaboratively with the team to ensure best practice & integrated client care

 

Skills and experience

Essential Selection Criteria:

  1. Empathy and patience for your clients and a passion for supporting people with disability
  2. Accreditation (APD) with Dietitians Australia (DA)
  3. Demonstrated ability as an independent worker who can exercise independent professional judgement on routine matters as well as working in a multi-disciplinary team for coordinated care.
  4. Demonstrated high-level skills in the development of physiotherapy supports for a wide range of novel and complex conditions
  5. Excellent organisation and time management skills
  6. Excellent written and verbal interpersonal communication skills
  7. Strong administrative and computer skills
  8. NDIS Worker Check, National Police Check, Working With Children Check, valid drivers licence and vehicle
  9. A problem solver with a can-do attitude
  10. Experience working or volunteering within the disability community.
